Abstract

Background: The Australian Commission on Safety and Quality in Health Care introduced the first national Clinical Care Standard for peripheral intravenous catheter (PIVC) care in 2021. However, we don't know how well clinicians in emergency departments (EDs) understand and follow this Standard. Exploring this gap can help us develop strategies to improve PIVC practices.
Aim: To understand what factors influence ED clinicians' adherence to the PIVC Standard.
Method: We conducted semi-structured interviews with ED nurses and doctors at two Australian EDs in 2023, using purposive sampling to include a range of opinions. Data was analysed using the Behaviour Change Wheel.
Results: We interviewed twenty-five participants and identified main barriers such as a stressful environment, inadequate education and training, and the lack of a feedback system. On the positive side, recognising areas of improvement, believing in patient engagement, and a desire to enhance practice were key facilitators.
Conclusion: The gap between guideline and clinician adherence suggests a failure to implement the key recommendations. Thus, it is important to recognise implementing guidelines into practice is a challenging task and having the guideline alone is not going to change practice. Therefore, strategies could be implemented to overcome barriers to ensure guideline compliance and adoption in complex clinical environments.

Biography

Dr Grace (Hui) Xu is a Nurse Practitioner at Queensland’s largest Emergency Trauma Center in Australia. She also holds a concurrent role as a joint Implementation Science Research Fellow at the Queensland University of Technology/ Royal Brisbane and Women's Hospital. As a Clinician-Researcher, Grace is dedicated to the core belief that patients in emergency departments should receive medical treatment without any associated harm. Her work focuses on advocating and promoting evidence-based practices in patient care, with a particular emphasis on optimising insertions and preventing complications associated with vascular access devices in emergency settings.
